Medical assessment forms are meant to be able to evaluate individuals with physical, mental or other medical disabilities. This is required for persons to secure should they need the special attention or benefits such as a disability benefit or other privileges. The same is required for those who are in need of Medicaid assistance as well as for student assistance grants.
The information which would be required in these forms is rather intensive. Apart from the personal information of the individual, there is also the matter of the person’s capacity to function on daily activities. The following categories are required to be assessed by qualified medical providers:
- Physical Disability – this would include spinal injuries or disabilities, joint and muscle diseases.
- Visual Impairment – from visual field concerns to more serious conditions that may or may no longer be corrected by surgery.
- Hearing Conditions – if a person has gradual hearing loss or is in need of hearing aids, surgery, etc.
- Neurological Disability – brain-related medical conditions such as epilepsy, psychiatric or psychological conditions.
- Psychiatric Disability
It is also required for evaluators to make sure that they take note of the individual’s personal ability to function regular tasks in medical assessment forms. ADLs or activities of daily living play a key role in these assessments. A person’s capacity is measured based from their level of dependence in completing these activities. These would include getting dressed, bathing, eating, grooming, cognitive functions or learning and overall mobility. The same evaluation is required for those who may require skin care, bowel or bladder care and other transfers.
